Welcome to our new page. I look forward to communicating with you monthly regarding current health issues. This month we will begin with the immunization requirements for the 2008-2009 school year. I’m always available for any questions you may have. Please feel free to call me at 954-583-6100 ext. 248.

IMMUNIZATION REQUIREMENTS 2008-2009

All students entering Kindergarten – 12th grade in the school year 2008-2009 are subject to the following immunization requirements:

CERTIFICATE OF IMMUNIZATION FOR K-12
(
EXCLUDING 7TH GRADE REQUIREMENTS )
(DEPARTMENT OF HEALTH FORM 680-PART A-1)

Each child shall present or have on file a Certificate of Immunization (Form 680) for the prevention of diphtheria, pertussis, tetanus, poliomyelitis, hepatitis B, rubella, rubeola, mumps and varicella.

Immunizations are to be documented on form 680 and Part A-1 completed ONLY if  the child is FULLY IMMUNIZED.  Fully immunized means a minimum of the following:

  • Five doses of diphtheria, tetanus pertussis vaccine

  • Four doses of polio vaccine

  • Two doses of measles vaccine*

  • One dose of mumps vaccine*

  • One dose of rubella vaccine*

  • Three doses of hepatitis B vaccine

  • Two doses of varicella vaccine

*Preferably as a combined MMR 

ADDITIONAL REQUIREMENTS FOR THE FOLLOWING GRADES:

First to Seventh Grades:
Varicella Vaccine or documented date of chickenpox illness on Health
Form 680

GRADES 7th-12TH CERTIFICATE OF IMMUNIZATION SUPPLEMENT
FOR THE
7TH – 12TH
GRADE REQUIREMENT
(DEPARTMENT OF HEALTH FORM 680-PART A-2)

  • Three doses of hepatitis B vaccine

  • Second dose of measles vaccine (preferably MMR)

  • A tetanus – diphtheria booster  

Please have your pediatrician’s office complete the HRS form 680 with the above immunizations documented and mail or bring it to the school ON OR BEFORE AUGUST 18, 2008.
